When to See a Fertility Specialist? 8 Signs You Shouldn't Ignore

October 24, 2025

Trying to conceive can be an emotional rollercoaster. Month after month, the hope and subsequent disappointment can leave you wondering: "Is this normal, or is it time to ask for help?"

Many couples hesitate, thinking they should just "try harder" or "give it more time." But knowing when to seek professional guidance is one of the most powerful steps you can take on your path to parenthood. At Alpha Fertility Centre Philippines, we're here to help you understand the signals your body is sending.

Here are 8 signs that it's time to schedule a consultation with a fertility specialist.

1. You're Under 35 and Have Been Trying for a Year

This is the general rule of thumb from reproductive medical societies. If you are under 35 and have not conceived after 12 months of regular, unprotected intercourse, it's a clear indicator to get a basic fertility evaluation for both partners.

2. You're Over 35 and Have Been Trying for 6 Months

A woman's fertility begins to decline more significantly in her mid-30s. Due to this accelerated timeline, we recommend that women over 35 seek help after just 6 months of unsuccessful trying. Early intervention is key to preserving your options.

3. You Have Irregular or Absent Periods

A regular menstrual cycle (every 21 to 35 days) is a key sign of ovulation. If your cycles are consistently irregular, very long, or have stopped altogether (and you're not pregnant), it could point to conditions like PCOS (Polycystic Ovary Syndrome), thyroid issues, or other hormonal imbalances that affect ovulation.

4. You Have Known Medical Conditions

Certain conditions can directly impact fertility. If you have been diagnosed with any of the following, it's wise to see a specialist early:

  • Endometriosis or Adenomyosis

  • PCOS

  • Pelvic Inflammatory Disease (PID)

  • A history of fibroids or ovarian cysts

  • Cancer treated with chemotherapy or radiation

For men, a history of testicular injury, hernia repairs, or sexual dysfunction warrants an evaluation.

5. You've Experienced Two or More Miscarriages

The heartbreak of pregnancy loss is profound. Experiencing two or more consecutive miscarriages (known as Recurrent Pregnancy Loss) is a sign that you should see a specialist. We can investigate potential causes, such as genetic, anatomical, or immune factors, and create a plan to help you achieve a healthy, full-term pregnancy.

6. You Have Painful Periods or Pain During Intercourse

While some cramping is normal, severe pelvic pain, extremely heavy bleeding, or pain during sex are not. These can be symptoms of endometriosis or adenomyosis, conditions where uterine-like tissue grows outside the uterus, which can cause inflammation and scarring that affects fertility.

7. You Have a Known Male Factor Infertility Issue

Infertility is not a female-only issue; it's a couple's issue. If your male partner has a known low sperm count, poor motility, or abnormal morphology from a previous test, or has undergone a vasectomy reversal, it's important to see a specialist who can address male factor infertility with treatments like ICSI.

8. A Gut Feeling That Something Isn't Right

You know your body better than anyone. If you have a persistent feeling that something is off—even if you don't fit neatly into the categories above—trust your instinct. There is no harm in getting a check-up for peace of mind. The worst-case scenario is that you get a clean bill of health and can continue trying with more confidence.

Taking the First Step: What to Expect at Your Consultation

We know that walking through our doors for the first time can feel daunting. We aim to make it a comfortable and empowering experience. Your first consultation will involve:

  • A detailed review of your medical and lifestyle history.

  • A discussion about your cycle and how long you've been trying.

  • An explanation of the basic, non-invasive fertility workup, which typically includes a pelvic ultrasound for the woman and a semen analysis for the man.

This initial evaluation is not about assigning blame; it's about gathering information to create a clear picture and a personalized path forward.
